Have beneficiary(s) complete, sign and notarize the Irrevocable Assignment and Power of Attorney form. Many insurance companies require this paperwork to verify. Also have the beneficiary(s) complete and sign Insurance Company Claim Form (if required by that company). 3FFC will verify benefits
